ALL appointment fees and booking fees deposits are NON-REFUNDABLE & NON TRANSFERABLE! By continuing to book your procedure, you are acknowledging this policy and providing consent that this booking fee is NON-REFUNDABLE & NON-TRANSFERABLE!
FANCY SHMANCY PMU may charge your credit card on file for cancellation fees and any other  fees that occur. You understand and agree you will not dispute these charges.
Cancellations of same day or less than 24 hours prior to your scheduled appointment will  result in a 50% cancellation fee of the services booked.
If you have any medical issues that might be a contraindication to the procedure you are  interested in, please obtain medical clearance from your healthcare provider indicating that you are  “medically cleared for cosmetic tattooing while on your medications and with your health conditions”  prior to the actual procedure appointment.

PRIOR TO BOOKING AN APPOINTMENT PLEASE UNDERSTAND THE FOLLOWING:
- You must be 18 years or older for any procedure (a valid photo ID is required for filing).
- Absolutely no guests, kids, or pets allowed.
- We reserve the right to refuse any service or terminate a client at any given time.
- See Illinois Department of Public Health Code Section 797.400
- A valid credit card must be on file at the time of booking and before any services are performed.
- A $100 non-refundable and non-transferable booking fee is required in order to secure the appointment  for the procedure. This booking fee, in addition to the $50 initial consultation fee, will be deducted from  the total cost of your procedure.
*For example, if the total cost of the procedure is $400, then the remaining balance due at the end of the  appointment is $250. ($400-$150=$250) Appointments should be made roughly 1-2 months out. Only book a service if you are ready to schedule  your appointment.
All appointment fees and booking fee deposits are NON-REFUNDABLE & NON TRANSFERABLE. This is to protect our artists from no shows, last minute cancellations,  last minute rescheduling, etc.  By continuing to book your procedure, you are acknowledging this policy and providing  consent that this booking fee is non- refundable & non-transferable. Once a service is performed, NO REFUNDS will be given. Full payment is due at the end of  the procedure.

CANCELLATIONS CONTINUED
If you decide to reschedule your appointment, you must call to reschedule at least 3  business days prior to your appointment. Only 1 rescheduling is allowed without automatically forfeiting your booking fee. If more  than one rescheduling request occurs, the $100 booking fee will be forfeited and a new  booking fee of $100 will be required for the second rescheduling request. Any type of “no show” without advance notice will be charged 100% of the value of your  appointment. Please allow yourself ample time for inclement weather, travel, and parking. Arriving 15 minutes late to your touch-up appointment time will result in a late arrival  charge of 50% and the appointment will be cancelled and rescheduled. Arriving 15 minutes late to a 3 hour service appointment time will result in forfeiting your  $100 booking deposit and a new booking deposit will be required.  Permanent makeup services appointments must be cancelled less than 72 hours prior to  the scheduled appointment or there will be a 50% cancellation fee. Lash services appointments must be cancelled less than 24 hours prior to the scheduled  appointment or there will be a 50% cancellation fee. *Covid related cancellations must be cancelled 72 hours prior to the scheduled  appointment or there will be a 50% cancellation fee. NO-SHOWS will be charged 100% of the services booked.
Please note that we do not offer refunds, and results can never be guaranteed.

You are prohibited from receiving PMU services, if you:

are under 18 years of age

are pregnant or lactating

are taking accutane or steroids

are easily triggered post inflammatory hyper pigmentation

have diabetes

have glaucoma

have allergies to makeup or colors

have skin diseases such as psoriasis, eczema and undiagnosed rashes or blisters on the site that is to be treated

have uncontrolled high blood pressure or mitral valve disorder

Let me know if you have/had allergic reactions to topical anesthetics.

Other contraindications: AHAs (lactic, glycol citric, retinoid and tartaric acids) or any OTC (over the counter) creams and lotions for tone correcting, anti-aging and exfoliating can fade and discolor your permanent makeup.

Flu or Cold: Please stay home and reschedule your appointment if you have a cold, flu or sinus/respiratory infection.

Coumadin: Amount of bleeding and response to topical epinephrine will determine coverage per application. If excessive bleeding occurs, extra appointments/applications may be necessary for desired outcome or procedure may not be effective.

Juvederm or other "fillers": Get permanent makeup first or ask your juvederm tech for clearance.

Shingles: If you have shingles on your face, wait several months for it to clear before permanent makeup.

Eyelift or blepharoplasty: Wait about 3 months.

Microblading on oily skin:

With this kind of skin type the pigment shades easily and after healing strokes are faint. Microblading cannot be performed on people who have oily skin. This procedure can leave visible marks (scars) in oily skin because this skin type is a contraindication for this procedure. Also, oily skin tends to regenerate more quickly than other skin types, so it will cause fading.
